  • Survey study of challenging experiences after ingesting psilocybin mushrooms: Acute and enduring positive and negative consequences
  • KEY TAKEAWAY
  • The risk of physical harm, aggressive behavior, and enduring psychological distress from psilocybin use is significantly increased by factors such as high dosage, lack of physical comfort and social support, and longer duration of the experience, yet when used responsibly in a controlled, supportive environment, the incidence of such risks is extremely low.

Introduction

In a landmark study by Carbonaro et al. (2016), an online survey was conducted involving 1,993 individuals who shared their most psychologically challenging experiences, commonly known as “bad trips,” after consuming psilocybin mushrooms. The survey aimed to characterize the acute and enduring positive and negative consequences of these experiences.

Challenging Experiences and Risks

Remarkably, 39% of respondents rated their challenging experience with psilocybin among the top five most challenging events of their lives. A significant finding was that 11% of these individuals engaged in behaviors that put themselves or others at risk of physical harm, with risk factors including the estimated dose, duration, and difficulty of the experience, as well as the lack of physical comfort and social support during the experience. Additionally, a small proportion of participants, 2.6%, exhibited physically aggressive or violent behavior, and 2.7% sought medical help due to their experiences.

Enduring Psychological Symptoms

Among those whose experiences occurred over a year prior to the survey, 7.6% sought treatment for enduring psychological symptoms. Alarmingly, there were instances associated with the onset of enduring psychotic symptoms and attempted suicide. Despite these challenges, the study also highlighted that a vast majority of participants, 84%, found the experience beneficial in retrospect.

Implications and Recommendations

This study underscores the complex nature of psilocybin experiences, emphasizing the need for a supportive environment and the potential risks associated with unsupervised use. It also points to the importance of harm reduction strategies in minimizing the risks of challenging experiences and maximizing the potential benefits. This includes providing individuals with information on appropriate dosing, setting expectations, and ensuring a supportive environment during the experience. Additionally, the findings highlight the need for further research into the mechanisms underlying both the positive and negative effects of psilocybin, to better understand how to harness its potential benefits while minimizing risks.

Conclusion

In conclusion, while psilocybin mushrooms can offer profound psychological benefits, they also carry risks of challenging experiences that can have lasting impacts. This duality emphasizes the importance of harm reduction practices, informed use, and the need for further research to optimize the therapeutic potential of psilocybin.
